REACT

The REACT project works to provide a short term reablement service for people in crisis or recently discharged from hospital. The aim of the service is to maximize your independence and wellbeing. This is a mulita disciplinary team working closely with Adult Services, Community Response Team, District Nurses, Occupational Therapy and Physiotherapists.  
Goals will be set by the service user and our team, which are then recorded on a support plan. Your goals will change throughout the six weeks to meet your changing needs.
This service is not timed or has a any set duration of visit. This increases the flexibility of the service and ensures you have the time you need to build on your goals.

  1. • Improve Wellbeing

  2. • Support access to community facilities

  3. • Meal preparation

  4. • Medication support and administration

  5. • Personal care (washing and dressing)

  6. • Improving Mobility

  7. • Reablement

To use this service you must be referred by social services but if you do not meet their criteria we can offer support from our domiciliary homecare team. Please contact us on 02392 520011.
